THE ORAL HEALTH SAVINGS PLAN

Dr. Erik Ellingsworth’s Oral Health Savings Plan is designed to provide affordability and greater access to quality dental care. The OHSP is not an insurance plan and cannot be combined with any insurance plan or provider.

** The Dual plan is for a Husband/Wife or Parent/Child
*** The Family plan is for an immediate family of four. Each additional child is $275. The Family Plan includes children until the age of 18 or the age of 23 if enrolled in college full time.

New Patients - There is an additional fee of $69 to cover a comprehensive exam and necessary x-rays.

The Oral Health Savings Plan offers you:
No yearly Maximums
No deductibles
No claim forms
No pre-authorization requirements
No pre-existing condition limitations
No waiting periods (immediate eligibility)
The Oral Health Savings Plan includes:
2 cleanings and exams in a 365 day period
2 fluoride applications per year
One set of check-up x-rays (4 bitewings)

The OHSP also offers the following percentages off normal procedural fees:
Restorative (fillings) 20%
Periodontal (gum disease) 20%
Crowns 20%
Bridges 20%
Implants 15%
Denture/Partial Denture 15%
Emergency exams 30%
X-rays 30%
Extractions 20%
Root canals 15%


All savings are at Dr. Ellingsworth’s office only. Dr. Ellingsworth reserves the right to refer certain procedures to the appropriate specialists (extractions, root canals, periodontal surgery, etc.). These discounts do not apply to services provided by any dentist other than Dr. Ellingsworth.


The savings plan is non-refundable and non-transferable.


Membership fees will be reviewed annually and are subject to change without notice.

Procedural fees are at the discretion of Dr. Ellingsworth. All payments are due at the time of service, unless other arrangements have been made prior to treatment.

Care Credit may be used in conjunction with the Oral Health Savings Plan; however there will be a reduction in the percentage of savings by 10%.
